BALL - Bring a soccer ball to your games - U7, U8 size #3, U9-U12 size #4, U13-U19 & Adult size #5.
SCHEDULES - Schedules will be posted no later than Sunday July 27. Games will start Friday August 1. Pool games will be played Friday and Saturday. Some Bracket games will begin Saturday afternoon. Most Bracket games will be played on Sunday August 3
CHECK-IN will be Thursday July 31at Disney's Wide World of Sports® Complex.
GAMES will be Friday, Saturday & Sunday August 1-3 (weather contingency date Aug 4)
Bring the following to check-in at Disney’s Wide World of Sports® Complex (Only the application and Entry Fee should be submitted in Advance) At check-in you will receive Wristbands for each player, T-shirts for each player and a Disney Credential for each player and two coaches which will allow free entry into the complex. These items will only be given out when each team has submitted the necessary forms.
(1) click for APPLICATION/ROSTER - The Application you submitted defines your Official Roster for the tournament. If there are any changes to your original Roster (on your application), all you need to do is to Print a new application/roster fill out and bring it to check-in. You do not need to notify us in advance
(2) click for DISNEY SPORTS WAIVER FORM (coming soon) + COPY - for each player and coach - You must turn in a WAIVER FORM (plus one copy) for each player and coach, with signatures. All Athletes and coaches will receive a credential when they turn in the waiver form. Credentials MUST be used every time you enter the complex. Lost credentials will need to be replaced by purchasing a ticket at the Box Office!
(3) click for HOTEL FORM (coming soon) - Each team must turn in a hotel form, indicating where you are staying or if you are commuting.
(4) PROOF OF BIRTH DATE - you must present proof of age for each player. PLEASE KEEP PROOF OF AGE WITH YOU ALL WEEKEND IN CASE IT IS NECESSARY TO CLARIFY A PLAYER'S AGE. Proof of Birth Date IS one of the following:
* copy of birth certificate (
in English, this is a FYSA requirement also)
* passport * drivers license * state issued ID * Military ID Proof of Birth Date IS NOT:
(5)click for Verification of Recreational Status (pdf) (htm) - All U9 and Older Rec teams must turn this form in at check-in. Teams will not be allowed to play in U9 and older Rec Divisions unless this form is turned in with all signatures
click for MEDICAL RELEASE FORM - OPTIONAL only - We will not be checking medical release forms at any time during the tournament. However, Medical Release forms are recommended - they are important if a player under the age of 18, is injured and requires medical treatment and a parent or guardian will not be present to authorize treatment by a physician. |
