BALL - Bring a soccer ball to your games - U7, U8 size #3, U9-U12 size #4,  U13-U19 & Adult size #5.

SCHEDULES - Schedules will be posted no later than Wednesday July 25.     

CHECK-IN will be Thursday August 2, at ESPN Wide World of Sports Complex - 5:00 to 9:00pm.

GAMES will be Friday (starting at 8AM), Saturday & Sunday (starting at 7:30AM)  (weather contingency date Aug 6).  Pool games will be played Friday and Saturday.  Some Bracket games will begin Saturday afternoon.  Most Bracket games will be played on Sunday.

Bring the following to check-in at ESPN Wide World of Sports Complex

(1)  click for DISNEY SPORTS WAIVER FORM 2012 waivers coming soon- Waiver for CHILD   Waiver for ADULT (includes Coaches and players 18 years or older)   Each team must turn in an  ORIGINAL plus COPY of the Disney Waiver Form for each player and coach.  Make sure to print the CHILD waiver for under 18, and ADULT waiver for 18 years or older)  

FOR EXAMPLE:  If you have 6 players, you will have 12 player waivers - six originals and six copies.  If you have 3 coaches you will have 6 coach waivers - three originals and three copies. 

Failure to bring both original and copy, will significantly delay your check in process!  All Athletes and Coaches will receive a Disney Credential when the waivers are turned in at check-in. Credentials MUST be used every time you enter the complex.  Lost Credentials will need to be replaced by purchasing a ticket at the Box Office!   

(2) PROOF OF BIRTH DATE - you must present proof of age for each player.  PLEASE KEEP PROOF OF AGE WITH YOU ALL WEEKEND IN CASE IT IS NECESSARY TO CLARIFY A PLAYER'S AGE.

Proof of Birth Date IS one of the following:

    * copy of birth certificate ( in English or with an English Translation)

    * Verified player pass from your State organization (as in signed, photo, laminated and does not say unverified.

    * passport

    * drivers license

    * state issued ID

    * Military ID

Proof of Birth Date IS NOT:

    school ID

    insurance card

    note from a doctor or parent

    roster

    player pass that is UNverified

(3) click for VERIFICATION OF RECREATIONAL STATUS - All U9 and older teams entered in Rec Divisions must present this form at check-in.  Teams will not be permitted to participate if this form is not presented.

click for MEDICAL RELEASE FORM - OPTIONAL only - We will not be checking medical release forms at any time during the tournament.  However, Medical Release forms are recommended - they are important if a player under the age of 18, is injured and requires medical treatment and a parent or guardian will not be present to authorize treatment by a physician.

At check-in you will receive Wristbands for each player, T-shirts for each player and a Disney Credential for each player and up to three coaches, which will allow free entry into the complex. These items will only be given out when each team has submitted the necessary forms....
