This Application defines your OFFICIAL ROSTER for the tournament.  If there are any changes to this original Roster, you need to Print and Fill out a new APPLICATION/ROSTER and bring it to check-in at Disney’s Wide World of Sports® Complex, along with a Waiver Form and Copy for each player and coach, and other items. - check the Forms & Information Page for complete details. (do not send these forms and waivers in mail)
