COACHES

CHECK-IN is Friday night Oct 28, 6 to 7pm and at half time at the University of Florida Women's Soccer Match at James G. Pressly Stadium - Directions  SPECIAL EVENT -  7pm UF Lady Gators vs South Carolina, Lady Gators will be signing autographs & posters after the game.

At Check-in you will receive t-shirts for each player on the team.  The following forms must be brought to check-in.

1) WAIVER FORM

If you applied online, you MUST bring a WAIVER with player/parent signatures.  You have a choice of

         (a)   WAIVER FORM with player/parent signatures,   OR

         (b)   printable application which has the waiver on it.

2)   APPLICATION/WAIVER  If you  need  to change your roster from the one that was submitted as your application you must bring a final application/roster to check-in.  You can find a printable version, on the Challenge Sports web site at printable application    

3)  PROOF OF AGE

Make sure to bring PROOF OF AGE for each player to team check-in. (such as player passes,  copy of birth certificate, drivers license, etc.)

4) HOTEL FORM - Fill out this hotel form if you are staying at a hotel

5) MEDICAL RELEASE FORM - RECOMMENDED only,  not mandatory. The medical release form authorizes treatment of a minor child by a physician if the parent is not present.   We recommend that Coaches have medical release forms for players under 18 if their parent or guardian will not be at the tournament all weekend.

 BALL - each coach bring a ball - U6 U7 U8 size #3, U9-U12 size #4,  U13 and older size #5

SCHEDULES - The First Game for each team will be posted on the web site Thursday night (2 days before the tournament)
