Bring the Original + 2 copies to Check-in, filled out and signed by your District Commissioner.  Electronically Generated Roster from the FYSA database is acceptable without DC signature.   If you have guest players on your roster, each one will need a copy of the FYSA Guest Player Form

(one for each player) must be notarized and presented at Team Check-in.  You may use medical forms that you are currently using for travel, as long as they contain the same information and are notarized.

      2) player & coaches passes

      3) medical release forms

      4) hotel form if staying at a hotel.
